Register your test kit

Thank you for ordering your test with E Shop Health. To help the lab process your sample, your test kit needs to be registered. We've tried to make it easy to register your kit and it should not take more than 5 minutes, but if you have any questions, please feel free to call us on 01625 568 188

To register your test kit please complete the following details

Test Kit Number	ESH- The test kit number starts with ESH and is a numerical 10-digit number that can be found on the back of your test kit or on your patient request form.
Patient ID	Please enter your unique Patient ID that can be found on the Introduction sheet that came with your test kit. It consists of both letters and numbers.
